Specialty Services   

The purpose of the Families and Children’s Educational Services program is to increase literacy levels and assess children’s medical and social needs prior to entering school. New parents receive home visits, medical care, childcare and mental health services. 

Men are fathers, brothers and sons we care about. Adult Medicine, family Planning as well as other services have a component for men as well as women. 

Seniors are an important segment of our community and, as such, deserve first-rate health care from providers who understand their unique concerns. Horizon offers seniors this quality care, as well as social and advocacy services. 

In addition to our medical/dental services, Horizon offers a variety of social service programs which provide the support individuals need to become or remain healthy. 

One high school in Jersey City now have school-based services. This made possible by the Jersey City Board of Education in conjunction with Horizon Health Center. 

Horizon has programs for individuals including teens who are incarcerated in our county penal institutions. We work with them to help them start new lives when they return to their neighborhoods. 

At Horizon we believe your health care needs go beyond standard medical treatment. That’s why we offer numerous resources to provide for your well-being. 

Provided by federal, state, county, city funds, patient fees, corporations, foundations,  and private donors.
