As a Federally Qualified Health Center (FQHC), Horizon Health Center sees all patients - whether you have insurance or not.

Horizon accepts various forms of payment including cash, checks, Mastercard and Visa. A minimum deposit is due when you check-in prior to your appointment. If you are unable to pay your full fee on the day of your visit, our staff will work with you to develop a payment plan, and will gladly answer any questions you may have.
