1.  What is Horizon Health Center?     A Community Health Center with Medical services for the whole family.
2.   How much do I have to pay?     Horizon takes Medicaid, private insurance and the uninsured. If a person has no insurance, we have a fee schedule based on income.  No one is turned away.  
3. If I don’t have insurance, can I still come to Horizon?  

Yes. All are welcome. 

4.   Does Horizon take Medicaid and Medicaid HMO’s?    Yes.  
5.   Does Horizon take Medicare?    Yes. We love seniors.
6.    Where is Horizon located?   

We have two locations.  1.  714 Bergen Avenue – near Montgomery Street at McGinley Square and 2. 412 Summit Avenue at Journal Square by the Motor Vehicle Building.  

7. Is there public transportation? Yes. See Directions Page.
8.    Who can come to Horizon?     Everyone! Men, Women, Pregnant Women, Teens, Children, Babies, Infants – Young and Old, Rich & Poor.  
9.    Does Horizon have Dental Services?    Yes, we have a beautiful dental suite.   
10.    Do I have to have an appointment?    It is much better to make an appointment. We do accept walk-in patients.  
11.    How do I make an appointment?     Call 201-451-6300 to make an appointment at 714 Bergen Avenue or (201) 963-5774 to make an appointment at 412 Summit Avenue..  
12.   Is there parking at Horizon?   

Yes. There are two municipal parking lots.  One on Bergen Avenue just South of Horizon and one on Montgomery Street just North of Horizon. 

13.    What kind of Medical staff does Horizon have?    OB/GYN,  Pediatricians, Family Practice, Nurse Practitioners, Nurse Midwives, Dentists, Registered Nurses, Licensed Practical Nurses and many more professionals.  
14.  What if I don't speak English?    Horizon has a multi-lingual staff. Spanish, English, Arabic, Tagalog, Russian and Urdu are some of the languages spoken. We also can arrange for an interpreter in most languages.
