If you have already enrolled in the Patient Portal you can login.

Please note that access to the Patient Portal is currently limited to adults over 18 years of age. This limitation is due to the State of New Jersey Department of Health Minors’ Rights regulation that allows children under the age of 18 to obtain confidential services.

Your health is our number one priority, and the HHC Patient Portal is your resource to your health information. The portal allows you to see your health information, make appointments, request refills, and even send and receive messages from your health care provider. You can access the portal from any internet connection or from your smart phone and send your requests at any time.

You can request for your Portal to be activated at any office visit, or request access using the “Pre-Register” button on the login page.

If you are requesting access online, our Patient Services staff will contact you to verify your information, and provide your log in name and password. Do not share these with anyone.

What do I need to sign up?

To sign up for the patient portal, all you need is an email address. It’s highly recommended that you NOT use a company-issued email from your job, and use a personal email instead.

Why can't I get a screen name instantly?

Horizon Health Center takes the security of your health information very seriously. By contacting patients requesting a portal, we ensure that the only one accessing your information is you.

Can I make an account for my other family members?

Each patient must use a different email address. Parents may request access to their children’s portal, but requests for minors may only be done in person at one of our offices, and a consent form must be signed.

Can I use the portal if I have an emergency and the office is closed?

The portal is not an emergency service. If you have an urgent need, call our main number – 201-451-6300. Even when the office is closed, team members are standing by to take your call, and can connect you with your doctor.
