With or Without Insurance, Horizon Cares for You and Your Family

As a Federally Qualified Health Center (FQHC), Horizon Health Center sees all patients - whether you have insurance or not. Uninsured patients pay a deposit on the day of your visit (similar to a co-payment). The remainder is on a sliding scale based on your individual needs. Our staff will be glad to work with you to help you apply for our FQHC discount.
