Horizon is now at Christopher Columbus Drive

Horizon Health Center has opened its offices at 115 Christopher Columbus Drive in Jersey City. These new facilities will provide services on two floors for Women's Health, Pediatrics, Internal Medicine, Dentistry and Infectious Disease.
If you previously saw our providers at our Summit Avenue office, we encourage you to visit our new facility. Of course, new patients are always welcome. We are also glad to announce that Dr. Han has joined our team, and she will continue to see her patients at our facility. Women's Health Renovations at Bergen Ave Office

After a brief relocation, our Women's Health Department has returned to our main office at Bergen Avenue. Our newly renovated offices on the second floor of 714 Bergen are bright and airy and ready for patients. Patients who wish to see Dr. Potter, Dr. Nilufar and the rest of the Women's Health team should call our office to make an appointment. 201-451-6300.
