From newborns to geriatrics, Horizon Health Center offers primary care services that ensure that our patients care continues throughout life. With yearly health maintenance visits, up-to-date immunizations, curative treatments, early detection and preventative services, we give our patients the tools for a healthy life. We practice an integrated approach to patient care by coordinating with sub specialists as needed and following up with patients to ensure that they are getting the appropriate care that they need. It is important for patients to note that once they have established a doctor at Horizon Health Center as their “Primary Care Physician,” they should communicate with their doctor on a regular basis. This means registering for a check-up every 6-12 months, calling your doctor after visiting a specialist or going to the emergency room, and making sure to bring all of your referral papers and other documents so that your doctor can evaluate them.
