    Horizon Health Center is a Federally Qualified Health Center in Hudson County, NJ that offers a comprehensive range of healthcare and educational services to care for a diverse and dynamic population. Horizon was established in 1963 as a family planning clinic and its legacy in the community is in Women’s Health. The health care center now has two sites based in Jersey City, NJ and one site in Bayonne, NJ that serve 19,000 patients and 60,000 visits per year.

    Our mission is to provide you with safe, highest quality, comprehensive, primary and preventative, healthcare services, social services and health education. To operate in cost efficient manner that maximize revenues while promoting a culture of accountability, respect and dignity. To be responsive to the needs of the entire community regardless of race, economic status, age, gender, or sexual orientation with continued emphasis on the underserved and those without access to care.
